Bear Bailey has been in love with his childhood best friend, Nikki Freeman, for as long as he can remember. The two work together at a small music store alongside their friends Ian and Sarah, but Bear has never had the courage to tell Nikki how he really feels. Already emotionally drained after his cat Sandy dies from accidentally eating oxycodone pills, Bear reluctantly agrees to go out with Nikki one night. While looking for a gift for her in a strange little occult shop, he buys a novelty trinket called a “One Wish Willow,” which supposedly grants a single wish when snapped in half.

Later that night, after dropping Nikki off at home, she directly asks Bear if he has feelings for her. Terrified of ruining their friendship, he denies it. Alone and angry at himself, Bear breaks the willow and wishes for Nikki to love him more than anyone else in the world.

Nikki suddenly returns to Bear’s house, emotionally vulnerable and claiming her father is dying of cancer. She asks to stay the night, and the two finally become intimate, but the moment quickly turns unsettling when Nikki abruptly screams and recoils from him for no clear reason. The next morning, her behavior grows stranger. She digs Sandy’s dead body out of the trash and creates a disturbing little memorial for the cat. Nikki brushes it off by claiming she was high on MDMA, then confesses that she’s always loved Bear. Before long, the two officially become a couple, leaving Ian and Sarah confused by how sudden and intense the relationship feels.

Ian eventually tells Bear that Nikki had openly said she wasn’t interested in him only days earlier, and that her supposedly dying father is actually perfectly healthy. When Bear confronts Nikki, she becomes deeply upset until he promises never to question her again.

From there, Nikki’s love becomes increasingly obsessive and terrifying. One night she watches Bear sleep and accuses him of not loving her enough. The next morning she literally duct tapes the front door shut to stop him from leaving. Bear still manages to get to work, only to discover Nikki packed him a sandwich made from Sandy’s remains.
As Bear’s fear grows, Ian admits he no longer wants Nikki around because of how unstable she’s become. Bear tries contacting the company behind the One Wish Willow, hoping to reverse the wish, but customer support chillingly explains that the spell only ends when either Bear or Nikki dies. Before hanging up, the operator asks if Bear wants to speak to Nikki — and all he hears is her screaming in agony.

Despite Ian’s objections, Nikki tags along to a party that night. During a drunken game of Jenga, she horrifies everyone by telling a grotesque and violent version of “Hansel and Gretel.” When Bear is dared to kiss Sarah during the game, Nikki forcibly takes Sarah’s place and kisses him herself before suddenly smashing a bottle into her own face in a violent outburst. Moments later, she acts as though nothing happened. Later that night, she threatens suicide to manipulate Bear into staying with her.
Soon after, Sarah secretly asks Bear to meet her alone. As Bear tries sneaking out, the “real” Nikki briefly resurfaces, begging him to kill her and end whatever the wish has done to her mind. Bear refuses. During the drive, Sarah reveals that she and Ian had casually slept with Nikki for years, and she subtly hints at her own feelings for Bear. Before the conversation can go further, Nikki appears out of nowhere, smashes the car window, and brutally kills Sarah by slamming her head into a brick. Calmly covered in blood, Nikki reminds Bear that this is exactly what he wished for.
Desperate to undo the nightmare, Bear buys the last remaining One Wish Willows from the occult shop. Since he already used his own wish, he begs Ian to use one to reverse Nikki’s obsession. Ian thinks the whole story is insane and instead wishes for a billion dollars. Instantly, cash begins pouring from his ceiling, proving the wishes are real and crushing Bear’s last hope.

With nowhere left to turn, Bear returns home hoping Nikki will undo the wish herself. Instead, he finds Sarah’s mutilated corpse displayed in the house while Nikki wears Sarah’s clothes. Armed and emotionally unstable, Nikki threatens both Bear and herself. Ian arrives unexpectedly, and Nikki immediately shoots him dead.
Completely broken, Bear decides the only way to end the curse is to kill himself. He swallows a fatal amount of oxycodone but almost immediately regrets it and tries to force himself to vomit the pills back up. Before he can, Nikki uses the final One Wish Willow on him, placing him under the same obsessive control she suffered through. Bear emerges from the bathroom, kisses Nikki lovingly, and then collapses dead from the overdose.

Devastated, Nikki prepares to shoot herself too — but the instant Bear dies, the wish finally breaks. Nikki suddenly returns to her normal self and realizes the horror around her: Ian dead on the floor, Sarah mutilated nearby, and Bear lifeless in front of her. She breaks down screaming in grief as the full weight of everything she’s done crashes over her.
