Plot Synopsis

College friends Sara, Max, James, Adrienne, and Greg travel to Thailand for a vacation. Although Sara and Max are best friends, Sara constantly finds herself overshadowed by Max's domineering personality and struggles to speak up. While partying aboard a yacht, Sara is accidentally left behind in the ocean and is severely stung by a swarm of jellyfish.

Despite Max being responsible for abandoning her, Sara quickly forgives her. The following day, the group joins a guided dive through an underwater cave system known as the Devil's Mouth. Feeling guilty about the previous day's incident, Max secretly pays their guide, Wat, to keep an extra eye on Sara, leaving Sara feeling patronized when she discovers the arrangement.
As the group ventures deeper into the caves, Max pressures Wat into leading them along a more dangerous route. They soon realize seawater has flooded sections of the cave, killing much of its freshwater ecosystem. After Sara discovers evidence that the dive supervisor has been attacked, including his damaged diving mask, Wat urges everyone to turn back. Max refuses, insisting they continue, sparking an argument that ends with Wat abandoning the group.
Moments later, a bull shark attacks, killing Greg. The survivors retreat to a rocky ledge, but Adrienne slips into the water and is also devoured. Using Wat's map, Sara devises a safe escape plan, but Max dismisses her idea and forces the group down another route. Meanwhile, Wat himself falls victim to the shark.

Sara, Max, and James flee through a maze of flooded tunnels with the shark in pursuit. Reaching a small air pocket, James becomes trapped beneath a collapsing rock and drowns. Sara and Max eventually surface, only to discover Max's route has led them in a circle back to the shark's hunting ground.

Frustrated, Sara finally stands up to Max and takes control, proposing they wait for the incoming tide so they can escape through an opening leading to the sea. As they prepare to climb out, the shark attacks again. Max, hampered by an injured hand, cannot pull herself to safety, and despite Sara's desperate attempt to save her, she is killed.
Now completely alone, Sara remembers Wat's warning about the cave's unstable walls. Using the last of her equipment, she lures the shark into charging at her before triggering a cave collapse that crushes the predator beneath tons of rock. Exhausted but alive, Sara climbs out of the cave and stands overlooking the ocean, bloodied and screaming in triumph after surviving the nightmare.
