While watching a horror movie, teenager Tuesday Campbell is attacked by Ghostface and left hospitalized. When Tuesday’s sister, Sara, and her boyfriend, Jack, learn of the attack, they realize the nightmare may be starting all over again.
Seeking answers, they visit their mother, Cindy Campbell, who has spent years living as a recluse, obsessively preparing for the day another killer would emerge. Sara and Jack ask for her help, convinced Ghostface has returned.

At the hospital, Sara visits Tuesday, only to receive a chilling phone call from Ghostface. The killer soon appears and launches an attack, but an unlikely hero—a nurse armed with a lunch tray—manages to knock him unconscious and save Sara.

Desperate for leads, Sara and Jack turn to Doofy Gilmore, the disgraced former sheriff who was once revealed to be the original Ghostface. Though reluctant, Doofy agrees to help investigate the new murders.

The survivors gather at Brenda Meeks’ house, where Cindy reunites with Brenda, her brother Shorty, and Brenda’s longtime boyfriend Ray Wilkins. Tensions quickly flare when Sara accuses Cindy of making everything about herself, causing her to storm out.

Meanwhile, Ghostface claims two new victims: Sheriff Greg Phillipe and his son, Jess. At the crime scene, reporter Gail Hailstorm reunites with Doofy as the investigation intensifies.

Ghostface then captures Gail, injecting her with a mysterious substance known as “The Stuff” before murdering her. Realizing the danger is escalating, Cindy and Brenda attempt to gather the remaining survivors, but Sara refuses to listen and decides to leave town with Tuesday.
The body count continues to rise when Brenda’s children, Dei and Brad, are murdered. Cindy desperately tries to warn Sara that the killers are closing in, but her warnings fall on deaf ears.

Eventually, Ghostface is unmasked as two people: Jack and Val, Brad’s girlfriend. They reveal that their motive is simple—they want to replace the aging legacy characters and become the new faces of the franchise.
Their victory is short-lived.

Before they can finish their plan, two additional Ghostfaces appear and kill both Jack and Val. The new killers unmask themselves as Anthony Anderson and Shaquille O’Neal. However, they too are quickly eliminated by yet another pair of Ghostfaces: Shorty and Ray.

Shorty and Ray reveal that they have secretly been behind everything. Bitter that Cindy and Brenda made the third and fourth movies without them, they orchestrated the bloodshed as revenge. Despite their grudge, they decide to spare Cindy and Brenda.

When Sara and Tuesday suggest that they should headline future installments of the franchise, with Cindy, Brenda, Shorty, and Ray serving in supporting roles, the four friends react with horror. Refusing to let anyone replace them, they tie Sara and Tuesday up inside the house and leave them to burn, declaring that no one will ever take over the franchise from them.
