Michelle Fuller, the CEO of pharmaceutical company Auxolith, finds herself abducted by conspiracy theorist Teddy Gatz and his neurodivergent cousin, Don. Held captive in the basement of their shared home, Michelle learns of Teddy's conviction that she is an "Andromedan," an alien species that has infiltrated humanity. Teddy, intending to use Michelle as leverage to demand the aliens' departure from Earth, shaves her head to prevent communication with her supposed mothership.
Despite Michelle's attempts to bargain, threaten, and even play along, Teddy remains steadfast. He informs her she has four days to negotiate a meeting with the Andromedans before an impending lunar eclipse, which he believes will allow the Andromedan mothership to enter Earth's atmosphere undetected.

Through flashbacks we learn that Teddy's mother, Sandy, became comatose after being a test subject for an Auxolith drug. Michelle covered up the incident and now funds Sandy's treatment. A police hunt for Michelle begins, while Teddy remains an Auxolith warehouse worker. Teddy has a tense encounter with Casey, the local sheriff who previously molested him and now seeks to make amends. Meanwhile, Teddy and Don continue to interrogate Michelle, but she refuses to confess to being an alien. Michelle, noticing Don's apprehension, tries to convince him to free her.
Teddy believes Michelle is an Andromedan royal due to her high pain tolerance during torture and brings her to dinner. A fight breaks out, and Michelle stabs Teddy with a fork. Casey arrives, investigating Michelle's phone signal, while Don takes Michelle back to the basement at gunpoint. Teddy distracts Casey with his apiary, blaming the Andromedans for its destruction. Michelle offers to help Don if he calls the police, and Don seemingly agrees before shooting himself. Teddy, hearing the gunshot, kills Casey and returns to the basement.
Michelle convinces a desperate Teddy that a bottle of anti-freeze in her car is a unique Andromedan medicine, claiming it will revive Sandy. Teddy races to the hospital and injects Sandy's IV bag with the anti-freeze, which kills her. Meanwhile, Michelle escapes her restraints and discovers that Teddy has abducted and murdered seven other individuals he suspected of being Andromedans.

Teddy returns home, where an enraged Michelle seemingly admits to being an Andromedan. She reveals that Andromedans arrived on Earth during the time of the dinosaurs and inadvertently caused their extinction. Feeling remorse, they created new species to populate Earth, which evolved into humans. However, they became disheartened when humans began to fight amongst themselves. The experiments performed on Sandy were intended to facilitate human evolution to the next stage, in accordance with Andromedan plans. Michelle then agrees to arrange the meeting between Teddy and the Andromedans at Auxolith headquarters.

Upon their arrival at the office, Teddy reveals a suicide vest. As confused colleagues alert the police to Michelle's return, she directs Teddy into her closet, claiming it to be the teleporter's location. The vest detonates as he enters, killing Teddy and rendering Michelle unconscious.

Michelle awakens in an ambulance on the night of a lunar eclipse. She flees and returns to her office, entering the closet. This action teleports her to the Andromedan mothership, confirming her identity as the Andromedan empress. After consulting with her peers, she concludes that the human experiment is a complete failure. Popping a bubble over a model of Earth, she causes the instant death of every human on the planet. Michelle then gazes at the now humanless Earth as bees slowly return to Teddy's colony
