Plot Synopsis

The year is 1968 and Iris Campbell along with her fiance, Paul, are attending the grand opening of a brand new restaurant: Sky View Restaurant Tower. At the opening, Iris suddenly has a strange premonition where one of the glass shards of the magnificent chandeliers cracks the glass floor the guests are standing on.

She also sees a chain reaction: the gas heater leaking and igniting a huge explosion, which together with the chandelier causes the tower to collapse, killing everyone. She manages to interrupt the dance party to get the guests off the floor before it breaks, saving the guests from the explosion, which now does not happen.
Now fifty-six years later, Stefani Reyes, a college student, is suffering from a series of nightmares about the Sky View Restaurant Tower collapse. She believes that in order to move on from these nightmares, she must visit home to look for answers. She is greeted by Marty, her father, along with her brother, Charlie, with whom she has a strained relationship.

Together the family goes and visits their other family; Uncle Howard and Aunt Brenda, along with their children: Bobby, Erik, and Julia. Stefani asks her Uncle Howard about her grandmother, his mother, Iris. He explains that she was a paranoid recluse who was extremely overprotective of him and her mother, Darlene. Her mother abandoned the family because of this. Her Aunt Brenda gives her her grandmother's letters that she sent to Howard. Stefani then goes to visit Iris, who resides in a remote fortified cabin.
Iris explains to Stefani that she interrupted Death’s natural plan that night at the Sky View Restaurant Tower. In retaliation, Death began to stalk and kill the survivors and their descendants of that night. Iris has been recording all of Death’s movements over the years in a book. She doesn’t seem to believe Iris, but as Stefani begins to leave Iris, terminally ill, leaves her cabin to give her the book, but also to prove to Stefani about death, as a chain reaction causes a weather vane to impale Iris, killing her.

After Iris’s funeral, Stefani’s mother, Darlene, returns home, much to Stefani’s chagrin, as she resents her mother for distancing herself from her own children. Later, at a family BBQ, Stefani reads her grandmother's book and learns that someone named JB found a survivor who also defeated Death. During the BBQ, Howards steps on a piece of glass and through the events of a chain reaction falls into the path of a running lawn mower, which kills him. Later that night, Erik’s tattoo shop catches fire, but he survives thanks to his leather clothing, which prevented him from being badly burned.
Later, after Erik brags about his survival to Stefani, Julia gets stuck in a garbage can where she is then killed by a garbage truck. The family now fully believes Death is stalking them. However, Marty and Brenda are spared because they are not a part of Iris’s bloodline, which keeps them safe from Death’s grasp. It is also revealed that Erik survives because his mother, Brenda, had an affair.

Darlenne suggests visiting a hospital where the mysterious JB works. JB turns out to be Willam Bludworth, a survivor of the Sky View Restaurant Tower Collapse. William reveals two ways to beat Death; dying and then being resurrected, or taking someone else’s life. The last to die in Iris’s premonition, William accepts Death is coming for him now that Iris is dead. He leaves to face it on his own terms.

Erik gives Bobby a snack which contains nuts, which Bobby is highly allergic to. This is intended to kill him so that Erik can revive him with an EpiPen. This plan backfires as an MRI machine malfunctions, which rips out Erik’s piercing, crushing him against the machine. As a nurse investigates, a vending machine spring comes loose and impales Bobby.
Stefani, Darlene, and Charlie travel to Iris’s cabin in an attempt to keep Death out, but along the way the RV in which they are traveling crashes and Stefani’s seat belt jams. After a chain reaction is triggered, the cabin explodes into a large fire. Stefanni begins to drown and Darlens saves Charlie, begging him to save Stefani. Darlene is killed as a lamp post falls and divides her in two, killing her instantly. Charlie eventually manages to free Stefani and revive her with CPR.

Believing them to be safe, they attend Charlie’s graduation ceremony. In the middle of the ceremony two realize that Stefani was not actually dead, just unconscious. Suddenly, a train derails into the neighborhood and the logs it was carrying fall onto Stefani and Charlie, killing them both.
