Plot Synopsis

Asia Reeves and her sister Maria attempt to escape their abusive father, but a confrontation at a store leads to Asia shooting him. While Asia is arrested, Maria remains trapped in their father's custody.

A decade later, Asia infiltrates The Virgil, an elite New York high-rise, by posing as a new maid. The building manager, Lily, notes the structure's century-long history of housing the wealthy. That night, residents Sharon and Kevin lead a masked group to attack Asia in her quarters.

They are caught off guard when Asia reveals herself to be a highly skilled combatant, killing her assailants. Asia then confronts Lily to find Maria, who is also a maid there. When her attackers miraculously resurrect, Asia is forced to flee through the building's ventilation system.

During the pursuit, Asia uses a shotgun to dispatch Sharon, though she is eventually rescued by Ray, Lily’s husband. Ray reveals the Virgil is actually a satanic temple where residents achieve immortality through human sacrifice.

As they move through the building, they are unknowingly tracked by Sharon’s sentient eyeball. Upon finding Maria, Asia discovers her sister is complicit and refuses to leave; Asia knocks her out to save her. After an ambush by the cultists, Asia and Maria escape via a dumbwaiter, while Ray is captured by Lily and a regenerating Sharon.

In the kitchen, Maria explains her past—how she fled their father before his death and eventually found "comfort" at the Virgil despite its dark nature. As Lily orders a building-wide manhunt, Maria saves Asia from a cultist but is subsequently recaptured.

Asia fights back with a burning axe, slaughtering many of the cult members. The sisters attempt to reach a fire escape through the elevator shaft but are cornered. Lily reveals the path to immortality requires Maria to perform a sacrifice—specifically, the maid Asia replaced.

On the top floor, the cultists summon Satan, who manifests through a severed pig's head inscribed with the names of the immortals. The entity revokes Ray's immortality, leading to his permanent death at Lily's hands. When Maria is ordered to kill Asia to secure her own place, she instead writes Asia's name on the skin and kills herself. Now immortal, Asia ruthlessly dispatches the cultists.

She eventually overpowers a possessed Lily and douses the pig's head in bleach to erase the names, causing the cultists to succumb to their wounds. Asia sets the head ablaze and escapes with Maria’s body.

After meeting an associate, Asia places Maria in a car where the sister resurrects. Asia reveals she salvaged the piece of skin with Maria's name to ensure her survival. As the top floor of the Virgil is consumed by an explosion, the reunited sisters drive away into the night.
