Plot Synopsis

The film opens in Woodsboro, California, at the notorious Macher house, site of previous Ghostface killings in 1996 and 2021. Scott and his girlfriend, Madison, fans of the Stab movie series, are exploring the property, with Scott mentioning a persistent rumor that the original Ghostface, Stu Macher, might have survived his supposed death. Ghostface soon appears, murdering Scott and stabbing Madison before igniting the house with her inside.

Meanwhile, Sidney Prescott has built a new life in Pine Grove, Indiana. She is married to police officer Mark Evans, runs a coffeehouse, and is raising her children, including a teenage daughter named Tatum (after her late friend, Tatum Riley).

Sidney receives a disturbing call from Ghostface, who, upon agreeing to a video call, reveals himself to be an aged, scarred man claiming to be Stu. "Stu" threatens Tatum, claiming he is outside the theater where she is rehearsing a play. Sidney and the police rush to the theater, but Ghostface flees after killing two classmates: Hannah, who was playing a fairy, and Aaron, who was operating her safety harness.

Later that night, Ghostface ambushes Tatum in Sidney's attic. Mark attempts to intervene but is knocked unconscious in a fall. Sidney gets Tatum into a safe room and uses an alternate exit to escape the house. They are chased until Gale Weathers and her assistants, Mindy and Chad Meeks-Martin, arrive and kill Ghostface by hitting him with their car.

Unmasking the killer, Sidney recognizes him as Karl Gibbs, a suspicious man who had recently purchased coffee from her, and an escapee from a nearby mental institution. Investigating with Gale, they interview Marco, the institution supervisor. Marco identifies "Stu" as a recently released "John Doe" who was Karl’s neighbor at the institution and was found as an amnesiac drifter in California. Mindy suggests that because Ghostface attacks are always personal, Tatum should gather all her friends in one location. Sidney agrees to a televised interview with Gale to draw out "Stu," while Tatum and her friends meet at a tavern. Unbeknownst to them, Ghostface ambushes and repeatedly stabs Mark at home.

When the interview ends, "Stu" calls Sidney, warning her that he is outside the tavern, prompting her to leave. Tatum grows suspicious of her friend Ben after finding a laptop in his car running a deepfake test to alter his face into "Stu's." After knocking Ben unconscious, she encounters Ghostface and flees. Ghostface then murders Tatum’s friends Chloe and Lucas, and injures Chad and Mindy. Tatum is chased through the streets until Ben finds her, only to be killed by Ghostface. Tatum escapes to Sidney's coffeehouse. Sidney, alerted to Tatum's presence but still far away, calls her and instructs her to lock herself in the office and retrieve a gun from the safe. Following theinstructions, Tatum shoots Ghostface through a wall. However, upon leaving the office to shoot the killer in the head, Ghostface rises. Tatum flees again and finds a second Ghostface at the front door. Sidney finally arrives at the empty coffeehouse. "Stu" calls to announce that Tatum is being held captive at home.

Returning home, Sidney finds a televised message from "Stu." The figure's face cycles through deepfakes of previous Ghostfaces—Nancy Loomis, Roman Bridger, and even Sidney’s late friend Dewey Riley—to tell Sidney he is recreating the first murders: forcing a person to watch a captive loved one. Outside, Sidney finds Tatum tied to a chair with Ghostface holding a knife to her. Ghostface reveals himself as Marco, the mental institution supervisor, who used his IT skills to create the "Stu" deepfake. The second Ghostface arrives, dragging the wounded Mark, and unmasks to reveal Sidney's neighbor, Jessica. Jessica explains that after reading Sidney's autobiography, she believed Sidney's success was due to killing her tormentors, which inspired Jessica to murder her abusive husband.

Disappointed that Sidney disappeared and didn't even appear during the recent New York attacks, Jessica tracked Sidney down. She spent time at the mental institution, met Marco, and moved next door with her son Lucas, whom she just killed at the tavern, deeming him too much like her husband. Jessica explains her goal is to turn Tatum into the "new Sidney" by forcing her to endure a similar trauma. Mark manages to cut the tapes on Tatum's feet, allowing her to rise and attack Jessica as Sidney shoots Marco. Jessica flees. Sidney tells Tatum to call an ambulance for Mark and not follow, as she will pursue Jessica. Sidney and Jessica fight, a struggle that only stops when Tatum shoots Jessica. As Jessica rises once more, Sidney and Tatum repeatedly shoot her in the head.
