Plot Synopsis

Young Cameron "Cam" Cade celebrates as his team, the San Antonio Saviors, clinches the league championship. However, his excitement quickly turns to despair when Isaiah White sustains a violent, potentially career-ending injury while scoring the decisive touchdown.

Years after said encounter, rising football star Cam, encouraged by his family and girlfriend, trains for the league combine. His career is jeopardized when, during a late-night practice on an empty field, he suffers a head injury from an attack by someone in a goat costume.

Despite brain damage risks, Cam's agent, Tom, motivates him to attend the combine to pursue the Saviors' quarterback role. However, after experiencing concussion issues, Cam lashes out and refuses to participate. Tom, who is also Isaiah's agent, reveals that Isaiah is considering retirement and has offered to train Cam for a week at a remote desert compound to prove he's a worthy successor. Cam accepts and travels to the compound, where he encounters crazed Isaiah fans attempting to intimidate him out of replacing their hero.

At the compound, Isaiah subjects Cam to unsettling challenges, including a drill where a football machine repeatedly strikes a player's head. Unbeknownst to Isaiah, Cam secretly receives injections of Isaiah's blood, believing it enhances his energy. Cam experiences hallucinations of masked figures, which he attributes to a concussion.

During a sauna session, Cam is attacked by a crazed fan but manages to overpower their leader. Isaiah then kills the fan, and Cam is disturbed by Isaiah's apparent lack of remorse. As Cam attempts to leave, Elsie, Isaiah's wife, apologizes and seduces him. The following morning, Cam awakens to Isaiah holding him at gunpoint. Isaiah mocks Cam's guilt, asserting that true killers feel no remorse.

Elsie lures Cam to a secret party where he's meant to meet the owners of the Saviors. Upon his arrival, the team doctor, Marco, silently signals for Cam to "run" before he departs. The owners then offer Cam a mysterious red liquid, which he consumes, unaware it is Isaiah's blood. Disoriented, Cam discovers Marco's decapitated head in a plastic-lined room. As he loses consciousness, Elsie offers comfort.

Cam awakens later in an ice bath at Isaiah's compound, an IV delivering Isaiah's blood into his system. He rips out the IV, seizes a jagged trophy, and resolves to kill Isaiah and escape. Cam locates Isaiah watching a fan-recorded video. Isaiah reveals that he is one of a lineage of manufactured "Greatest of all Time" (G.O.A.T.) players, whose enhanced powers are derived from ritual blood transfusions. These G.O.A.T.s must fight to the death for the title. Cam and Isaiah battle, with Cam ultimately killing Isaiah using a helmet.

Cam enters a football field to find Tom, Elsie, and the Saviors' owners, all wearing animal masks, awaiting his arrival to sign a contract. They reveal that Cam's father, manipulated by the elites, orchestrated events for Cam to become the next G.O.A.T., and that the mask attack was a setup to lead him to the Saviors. Cam retaliates, killing his attacker with a hammer, then the owners and Elsie with a sword. He spares Tom, but Tom is subsequently dragged into a pentagram and killed. Cam, half-bloodied, then walks past masked cheerleaders outside the compound and into the desert.
